Rehabilitation Support that
Moves You Forward

Forward Motion Rehab provides experienced Rehabilitation Support Workers to improve the quality of life and outcomes for individuals living with brain injury across Southern Ontario. We provide individualized support at home, in the community, at work, at school, or wherever support is needed to help clients move forward.

Who we support

Support for people living with brain injury

Forward Motion Rehab supports people living with the effects of brain injury, with a particular focus on individuals injured in motor vehicle accidents. Whether someone is recovering from a concussion, a more complex traumatic brain injury, physical injuries, or a combination of challenges, our focus remains the same: providing compassionate, individualized support that helps people build independence, confidence, and move forward in their recovery.

Brain injury affects people differently. Some common challenges include:

  • Memory difficulties
  • Reduced attention and concentration
  • Slower processing speed
  • Fatigue
  • Headaches, dizziness and balance problems
  • Difficulty planning, organizing and problem-solving
  • Changes in communication
  • Anxiety, depression, irritability or mood changes
  • Reduced self-awareness or insight into challenges
  • Sensitivity to light, noise or busy environments

Together, these can make everyday activities such as socializing, managing appointments, preparing meals, shopping, returning to work, driving, or maintaining relationships feel overwhelming. Forward Motion Rehab is here to help.

The forward motion difference

Our Approach

At Forward Motion Rehab, we believe meaningful rehabilitation happens when support is provided in the places that matter most.

Our experienced Rehabilitation Support Workers help bridge the gap between clinical rehabilitation and everyday life; supporting clients at home, in the community, at work, at school, and wherever support is needed. Each worker is thoughtfully matched with a client based on experience, education, location, cultural considerations, and shared interests. Our focus is to support individual needs, build strong relationships and facilitate meaningful, goal-focused rehabilitation.

Our Rehabilitation Support Workers provide non-regulated rehabilitation services under the direction and guidance of regulated health professionals and the rehabilitation team. Through careful screening, orientation, ongoing professional development, and adherence to applicable privacy, safety, and professional requirements, Forward Motion is committed to providing safe, reliable, compassionate support while working collaboratively with clients, families, caregivers, and rehabilitation teams.

Who we are

21 years of acquired brain injury experience: from front line to leadership

Courtney Conti brings more than 21 years of experience in the acquired brain injury, rehabilitation and accident benefits sectors, supporting people with complex rehabilitation needs across a wide range of community settings.

She began her career as a Rehabilitation Support Worker, working directly with clients and multidisciplinary teams on community reintegration, life skills, educational and vocational goals, and physical rehabilitation. Over two decades she advanced into supervisory, regional management, service coordination and director-level roles; experience that spans front-line delivery through to organizational leadership and strategic planning.

That progression is deliberate, and it shows in how Forward Motion operates: a hands-on understanding of what clients actually need, paired with the operational rigour that referral partners depend on.

Questions

Frequently asked questions

What is a Rehabilitation Support Worker (RSW)?

A Rehabilitation Support Worker provides one-on-one support to people recovering from injury or illness. Working under the direction of regulated health professionals, they help clients put rehabilitation goals into practice at home and in the community.

How is an RSW different from a Personal Support Worker (PSW)?

Both roles are valuable, but they do different types of work. An RSW focuses on rehabilitation such as skill development, cognitive strategies, community participation and the functional goals set by the rehabilitation team. A PSW primarily assists with personal care and activities of daily living.

Do Rehabilitation Support Workers work independently?

No. RSWs work under the guidance of regulated health professionals such as Occupational Therapists, Physiotherapists, Speech-Language Pathologists, Social Workers or Case Managers.

Where are services provided?

Services are delivered where rehabilitation happens naturally as a part of daily life - the client's home, community, workplace, school, gym or other meaningful environments. We serve clients across Southern Ontario.

What types of injuries or conditions do you support?

We primarily support people with acquired brain injuries following motor vehicle accidents. Depending on the referral, services may also suit people with neurological conditions or other complex rehabilitation needs.

Do you offer evening or weekend services?

Service availability depends on client needs and staff scheduling. Ask us about your situation and we'll tell you honestly what we can accommodate.

Are your services covered by insurance?

Many services are funded through Ontario's automobile insurance system following a motor vehicle accident. Private funding options may also be available.

Are you licensed to direct bill through HCAI?

Yes. Forward Motion Rehab holds a service provider licence with FSRA (the Financial Services Regulatory Authority of Ontario) and bills auto insurers directly through HCAI.

Do I need a treatment plan (OCF-18) before services begin?

Generally, yes. Rehabilitation Support Worker services are recommended within a treatment plan developed by a regulated health professional, then submitted to and approved by the auto insurer.

How quickly can services begin?

Timelines depend on funding approval, location and staff availability. We move as quickly as those factors allow and will give you a realistic estimate at intake.
